The phone rings. You are mid-adjustment. Your front desk is already on the other line. So it goes to voicemail.

Now answer this: how many calls did that yesterday?

Almost nobody can. And that, rather than the calls themselves, is the actual problem.

A missed call leaves no evidence

Every other problem in your practice leaves a trace. A cancellation shows up on the day sheet. A refund shows up in the numbers. A complaint reaches you eventually. A missed call leaves nothing at all.

  • It is not on your day sheet, because no appointment was ever made.
  • It is not in your practice software, which only knows about people who became patients.
  • It is not in your month-end numbers, which count what happened, not what did not.
  • It is not in anyone's memory, because nobody was there to hear it.

So the loss is real and the record is empty. Work that nobody measures does not get managed, which is why practices can go years without addressing something that touches new patients directly.

Do the arithmetic on your own numbers

You do not need an industry statistic here, and you should be sceptical of anyone who offers you one. You need two figures, both of which are yours.

First, what a new patient is genuinely worth. Not the first visit — the whole course of care. Average number of visits multiplied by average revenue per visit. Then discount it by the share of first-time callers who actually become patients, because not every call converts.

Second, how many calls you missed in one ordinary week. This is the number nobody has, and it is easier to get than most practices expect. Almost every phone system can export a call log. Ask for one week — not a holiday week — and count total inbound, answered, sent to voicemail, and arriving outside opening hours.

Multiply the two together and you have a figure built entirely from your own practice. It is an estimate, not a market statistic, and that is precisely what makes it worth trusting: you can check every input.

Be conservative with the inputs. A small number you believe will change your behaviour more than a large one you quietly suspect is inflated.

This is a coverage problem, not a staffing problem

The instinct is to conclude the team is not working hard enough. That is almost never what is happening. Your front desk is already busy, and the calls they miss are mostly the ones arriving while they are dealing with the person in front of them or the caller already on the line.

The phone rings when it rings — not when somebody happens to be free. One person covers roughly forty hours a week. Your practice number can ring across all one hundred and sixty-eight. That gap is structural, and no amount of effort closes it.

What actually closes the gap

An AI receptionist answers the calls that would otherwise reach voicemail, including the ones at twenty to nine on a weeknight. The part that matters is what happens next: it checks your real availability and books into the scheduling system you already use.

It does not replace Jane, ChiroSuite or ClinicMind. It writes to them. Your team keeps working from the same schedule they always have, and the booking is simply already there. When a call needs a person — anything clinical, anything sensitive, anyone who asks — it hands over rather than pressing on.

Before you evaluate any system, though, get your baseline. A practice that knows its own number can tell whether a change worked. A practice that does not is guessing either way.
